INSURANCE REQUIREMENTS

In order to rental equipment from Special Effects Unlimited, we will need you to provide and insurance certificate with the following information:

Your general liability coverage and policy number.

Your misc. equipment or property insurance and policy number.

Special Effects Unlimited named at the certificate holder.

Special Effects Unlimited named as additionally insured on the general liability and loss payee on the property/equipment.
